Au cours des manifestations de 2022, la province du Sistan et du Baloutchistan a connu une répression parmi les plus violentes exercées par les forces de sécurité iraniennes, y compris l'IRGC. Le 30 septembre 2022, le chef-lieu de la province, Zahedan, a été le théâtre d'un "vendredi sanglant", lorsque les forces de sécurité ont ouvert le feu à balles réelles sur une manifestation s'étant formée en marge de la prière du vendredi. Au moins 70 manifestants ont été tués par balles. Depuis lors, la violence à l'encontre de participants à des manifestations ultérieures s'est poursuivie. En tant que commandant du quartier général régional de l'IRGC pour les unités de l'IRGC engagées dans cette région, Mohammad Karami porte la responsabilité des violences perpétrées par l'IRCG contre les manifestants dans la province du Sistan et du Baloutchistan, notamment pendant la période du "vendredi sanglant".
